Blended Case Management
Blended Case Management is a voluntary program designed to assist individuals with mental health diagnoses to meet their mental health care needs and improve their overall wellbeing by helping them gain the tools, resources, and confidence they need to manage their life more independently.
Getting Started
Each individual enrolled into the program will participate in an intake that will include an initial assessment to determine what their needs and goals are. The service plan that they participate in developing is updated semi-annually and is used as a reference to track their goals and progress.

Blair, Bedford, and Somerset County clients have access to our Wellness Program. This program is designed to give extra support, resources, and advocacy for clients who have any physical health goals or extensive physical health diagnoses.
Blended Case Management
Individuals receiving Blended Case Management services are entitled to on-call services. In the event of an emergency that occurs outside of the hours of operation, a Blended Case Manager is available to assist you. Clients will also be informed by their BCM at intake how to access the Cambria County Mobile Crisis Unit if a psychiatric emergency should occur.
Blended Case Management services are available for adults, adolescents and children who are receiving Medical Assistance and meet eligibility guidelines. To qualify for BCM the individual must be diagnosed with a psychiatric disorder and may also have:
- 6 or more days psychiatric inpatient
- Met criteria for involuntary treatment
- Missed 3 or more mental health service appointments
- 2 or more face to face visits with the Mobile Crisis Unit
- Receiving, or in need of services from 2 or more human service agencies.
As with any program, there are limitations on what the program/case manager can provide. Blended case managers cannot diagnose, provide medical or legal advice, or transport clients. Blended case managers will enforce healthy therapeutic boundaries and maintain confidentiality. The Blended Case Management program does not directly provide financial assistance, housing, etc…, but every effort will be made for proper resource coordination and advocacy to assist with these needs.
